A misaligned frame is about the reveal inside the frame itself becoming uneven, the gap between the door or sash and its own frame. A gap around a window or door is different: it's the separation happening at the boundary between the frame unit and the wall structure it's set into, the trim pulling away from the siding, or a widening seam between the window unit and the interior drywall.

It often shows up at the top corner of the opening first, where a settling wall section has pulled down and away from the frame above. This is the frame and the wall around it physically separating, and it often shows up as a draft, a sliver of daylight, or a spot where water starts getting in.

It can be, especially if the gap is small, hasn't changed in years, and shows no other symptoms nearby. But beyond the comfort issue of a draft, a gap at a window or door is a point where wind driven rain can work its way into the wall cavity, leading to hidden moisture damage that isn't visible until it's more advanced. Combined with any of the other signs in this silo, sticking doors, sloping floors, or cracks radiating from nearby corners, a widening gap points to foundation settlement pulling the wall away from the window or door unit rather than normal seasonal building movement.

Recaulking or re-trimming closes the gap temporarily but doesn't stop it from reopening if the wall is still moving. If the underlying cause is foundation settlement, the gap will widen again on the same schedule, and you'll be back to recaulking every year or two. Because that's where a settling wall section pulls furthest away from the rigid frame above it.
